  • Please contact the school nurse if your child has health considerations that require extra organizing, staff education, and preparation while attending school. Parents, the school nurse, teachers, and other school staff will work together to create a plan to keep the student healthy and safe and to minimize illness related absences from school. Please provide information from your child's healthcare provider explaining your child's health needs as well as written orders for medications, treatments, procedures, or medically indicated dietary or activity restrictions.

  • Ohio Revised Code requires all students attending preschool to have an up-to-date health physical on file in the school health record. For students younger than 3 years old, the physical must be within the past 6 months or sooner. For students 3 years old and over, the physical must be within the last 12 months.

    Immunization records are required as well.

  • Please review the information posted here regarding required immunization updates for 7th and 12th grade. You can "beat the rush" by addressing those updates with the healthcare provider at your child's regularly scheduled health physical in the year prior to them entering 7th and 12th grade. Turn in updated immunization records to the school clinic to be up-to-date and ready to go when your child's 7th and 12th grade year begins.

  • Please keep students home from school for...

    • fever in the past 24 hours
    • vomiting in the past 24 hours
    • diarrhea in the past 24 hours
    • uncontrollable coughing
    • red and draining eyes
    • undiagnosed skin rash

    Please notify your child's school office when your child will be absent from school. Please contact the clinic staff with any questions in regard to student illness, staying home from school for illness, and school policy for student illness.

    If you take your child to the healthcare provider for evaluation, please ask for a note to excuse the absence from school. Turn absence notes in to the school office.

  • Every fall as required by ORC, students in grades PS, K, 1, 3, 5, 7, and 9 are screened for vision issues. Students in PS, K, 1, 3, 5, 9, and 11 are screened for hearing deficits. Screenings will be done during the month of October with screening complete by Nov. 1. Parents will be notified if their child has problems with the vision or hearing screening. A referral will be sent to parents for follow up with the child's healthcare provider or a vision or hearing specialist.

  • In the spring each school year with parent/ guardian permission, 5th and 6th grade students have the opportunity to participate in puberty lessons. The girls lessons are facilitated by the school nurse and female teachers. The boys lessons are facilitated by a male health teacher and male teachers.
